Reservation for Conference Room

  • Please remember to fill out the Attendance by scanning the QR CODE. This helps us to keep track of room usage.
  • Bonafide Faculty, Staff,  and Students can make reservations.
  • Always follow the “CLAYGO” (Clean As You Go)  After using the conference room.
  • The room has a maximum Seating Capacity of 15 (seats). Ensure that you do not exceed this limit.
  • Use the government equipment with care.
  • Only those with approved letters are allowed to use the facility.

  • The reservation is two days before the scheduled event. 
  • For classes, the professor must send their Letter of Intent with detailed information to our office email and address it to the University Librarian.
  • For student Groups or Student Organizations in their needs for practices, film viewing, general assembly, and other academic activities, they must send a Letter of Intent to use the space with the approval and endorsement of their Class Adviser and the Office of Student Affairs (OSA).
  • For seminars, culminating activities, and training/workshops, the organizer, either faculty, staff, or student, must send a letter that indicates the subject, number of participants, and objective/s of the event.
Note: “MAJOR EVENTS ARE GIVEN PRIORITY BY CONFERENCE ROOM. WE WILL INFORM BEFOREHAND IF THERE ARE ANY CONFLICTS WITH THE RESERVATION SCHEDULE.
